Comprehensive Psychiatric Diagnostic Evaluation
Online or in-person, this is the first visit with your practitioner designated for you to share your story and spend time creating a plan of care together.
We’ll spend time learning about you, hearing your story, and asking questions. we’ll want to know what you’ve been struggling with, your health history, the important people in your life, your habits and routines, and what you’d like help with. We’ll create a plan for you, which might include lab tests, recommendations on nutrition, activity, sleep, psychotherapy, supplements, and medications.

Psychiatric Medication Management 25 min Follow up
Follow up visits for medication management
We’ll look at what has come up since we last met. We’ll track your progress, review lab results, consider what’s working well for you and what isn’t, and refine your plan.
Online or in-person. This length of visit is best for focused concerns, or following up on medication or supplement management. If we are integrating psychotherapy into your treatment, a 45-minute session would be best suited for that.

Lab testing
Lab testing will be discussed in session and may be recommended depending on your treatment goals.
Routine labs: may be covered by insurance, when not covered prices vary greatly.
Specialized Functional Lab Testing: range $ 200 - $ 600, insurance may cover partially.
Lab Review Fee: $ 150
Psychiatric Medication Management 50 min Follow up
We’ll be able to do everything that’s included in the 25-minute follow-up, plus time for psychotherapy interventions or more extensive review of how the treatment plan is shaping up for you.

Insurance
Evolve Psychiatric Services is an “out of network” provider and doesn’t accept health insurance. We can provide superbills which you can submit to your insurance for reimbursement if you have a PPO.
For more information, we recommend asking your insurance about out of network benefits/reimbursement.
No Surprises Act
In compliance with the “No Surprises Act” starting on 1/1/2022, “Good Faith Estimates” are available upon request and will be provided to all patients within 3 business days.
